身分証明書の提出方法は、ご契約者様自身のご都合に合わせて弊社へ送っていただきます。
提出は、郵送、画像の送信、それ以外の場合はFAXでお送りいただけます。

​身分証明証は、原本ではなく複写したものをお送りください。



法人及び外国人のお客様

法人様及び外国人の方で、ご契約をご希望になる場合、次のお手続きが必要となります。

携帯電話不正利用防止法に伴い身分証明証の確認を行わせていただいております。なお、弊社はそのご提出いただいた身分証明証について一定の期間、そのご提出いただきた身分証明証の保管する義務があります。保管につきましては、弊社は個人情報保護法を適切に遵守いたします。

法人様のご契約際に必要な身分証明書類
1)登記事項証明書、印鑑登録証明書の写し
2）官公庁から発行、発給された書類その他それに類ずるもので、法人の名称および、本店等の所在地の記載があるもの。

身分証明証は、公的機関において、領収日付または発行、発給日から6か月以内のものでお願いいたします。なお、ご提出の身分証明証記載の住所、所在地が現在のものと異なり場合には、補完書類として、次のものを併せてご提出いただく必要があります。

1)他の本人確認書類
2)国税、地方税の納税証明書又は領収証書
3)社会保険料の領収証書
4)公共料金（日本の水道水、ガス、電気またはこれらに準ずるもの）の領収証書
5)日本の官公庁や外国政府及び国際機関が発行した書類で1～4に準ずる書類

法人情報とご契約のお申込み、お手続きをのご担当者の身分証明証も併せてご提出いただく事となります。この際の身分証明証は、個人のご契約者様の場合と同じなのでご確認頂きますようお願い申し上げます。

ご契約際には、事前にご自身でご確認の上、ご提出いただけますようお願い申し上げます。
ご提出いただいた、身分証明証は返却できかねますのでご了承くださいますようお願い申し上げます。

ご契約後、商品の郵送する場合には、規定により書留郵便による転送不要郵便物等の取り扱いでご提出いただいた所在地に郵送させていただきます。

Identification card for individual contractors

At the time of contract, we will confirm the identity of the applicant and at the same time confirm that he / she is the applicant. We apologize for the inconvenience, but we ask that you agree to submit your ID. The ID card to be submitted must be issued by a public institution, and must be within 6 months from the date of receipt and date of issue.

We will confirm your identity at the time of contract. We kindly ask for your cooperation when making a contract.

Since it is stipulated that the type of valid identity verification certificate is the "public certificate" issued by a public institution designated by the government agency, the "public certificate" that can be submitted as identity verification is stipulated. Please confirm the "proof" before submitting.
Please note that you cannot make a contract with an expired certificate.
The "public certificate" issued by a public institution designated by the government office must match the information of the contractor as of the time of the contract.

Please note that we cannot return the submitted items.

Please send us a copy of the ID you submit, not the original. After confirmation by the person in charge, we will proceed to the contract procedure.
In addition, since there are restrictions on refraining and copying such as my number and basic pension number by law, it is recommended to hide the individual number and basic pension number by yourself after copying. I will do it. When submitting a resident's card, please submit one without an individual number when receiving a certificate of matters to be stated in the resident's card.

After applying, it is an absolute requirement of the contract to be able to confirm the identity of the contractor, so when applying, we will use the type of "public identification card" that we can accept as an identification card. Please check and prepare in advance.

The ID card submitted by us at the time of contract is obliged to keep the contractor's information for a certain period of time in accordance with the Mobile Phone Unauthorized Use Prevention Law, so the ID card submitted is Please note that we will keep it for that period.
In addition, the personal information of the contractor submitted will be properly managed in accordance with the Personal Information Protection Law.
